Add glfwSetWindowMonitor
...
This adds the ability to switch between windowed and full screen modes,
move a full screen window between monitors and update its desired
resolution and refresh rate.
Fixes #43 .

Added GLFW_INCLUDE_VULKAN. Added glfwVulkanSupported,
glfwGetRequiredInstanceExtensions, glfwGetInstanceProcAddress,
